As of 2025, the population of Malaysia is undergoing a demographic transition, with 3.9 million residents aged over sixty constituting 11.6%. The myriad challenges faced by this cohort in accessing health services, chronic illness care, and maintaining social connections is challenging the healthcare system and family caregivers. Although some digital health solutions exist, senior acceptance is still limited due to the complexity of system fragmentation and improper integration. This paper discusses “Care Connect,” a user-centric mobile health app meant for senior citizens in Malaysia. The platform’s “ user-centric design” supports health management (medication reminders, mood tracking, and appointment scheduling), protective safety (hospital locator, emergency calls, and medical transport), and social integration (video/voice calls, messaging, and caring monitoring portals). “Care Connect” aims to reduce social isolation, improve health literacy, and help senior citizens live more autonomously. The mobile health software aims to assist with self-management of health conditions and chronic illness to facilitate independent living. The healthcare system and family caregivers are under stress because elderly citizens of Malaysia face difficulty receiving the needed care. Digital health adoption is still very low because of its complex interfaces. “The Care Connect" consolidates the social, safety, and health components into one platform for seniors. With the use of a carer monitoring site, it has communication channels, emergency systems, and medication reminders. It has social, medication, and health literacy benefits. It enhances independent living and improves health outcomes for seniors, which supports active aging. It has social and health benefits for the elderly and the platform supports the increasing demand for elder services. It has a universal model which allows for easy expansion throughout Malaysia and Southeast Asia. It has potential scalability for the entire region. The system can be integrated into the mHealth elder care ecosystem with mHealth synergies with health insurers and care providers.
